Study objectives | To determine: 1. The optimum methods of assessing the elderly (aged 75 years and over) in primary care as required by the GP contract (through identifying disease and need) 2. The optimum model of management of elderly participants identified by the above assessment process. The study will compare assessments at different levels (brief health interview and detailed health assessment) and by different methods of administration (postal, lay interviewer, practice nurse). Two different management models will be compared: the primary care team and a multidisciplinary geriatric team. |

Key inclusion criteria | All people on the GP register eligible for the over 75 check in the year following practice randomisation and training. |
Key exclusion criteria | Patients in long stay care, Nursing Homes or patients who are terminally ill. |
