Scientific title
Study objectivesTo determine:
1. The optimum methods of assessing the elderly (aged 75 years and over) in primary care as required by the GP contract (through identifying disease and need)
2. The optimum model of management of elderly participants identified by the above assessment process.

The study will compare assessments at different levels (brief health interview and detailed health assessment) and by different methods of administration (postal, lay interviewer, practice nurse). Two different management models will be compared: the primary care team and a multidisciplinary geriatric team.

Key exclusion criteriaPatients in long stay care, Nursing Homes or patients who are terminally ill.
