Plain English summary of protocol
Background and study aims
This study aims to understand the daily activities of general practitioners (GPs) in two regions of France. By collecting detailed information about their work, the study hopes to provide insights into how GPs spend their time and the challenges they face.
Who can participate?
General practitioners from Île-de-France and Nouvelle-Aquitaine regions can participate. The study will include both self-employed and salaried GPs.
What does the study involve?
Participants will be asked to complete an electronic questionnaire on a randomly selected workday. The questionnaire will cover all professional tasks performed that day, such as consultations, home visits, and administrative work. It can be filled out on a smartphone, tablet, or computer.
What are the possible benefits and risks of participating?
None
Where is the study run from?
Société Française de Médecine Générale (France)
When is the study starting and how long is it expected to run for?
March 2024 to December 2025
Who is funding the study?
The study is supported by the Regional Union of Liberal Physicians (URPS) and the National Federation of Health Centers (FNCS) in Île-de-France and Nouvelle-Aquitaine.

Study objectives | The organization of the healthcare system has undergone profound changes over the last 20 years, particularly with regard to the functions and conditions of general practice. Changes in legislation and in the agreements governing the liberal health professions have had a particular impact on funding, remuneration methods and levels, and the role of the various players involved. The issue of demographics and the territorial distribution of general practice has become a recurring theme. Other factors have also contributed to the evolution of practices and organizations: advances in medicine and surgery, the emergence of new health issues, the greater need for coordination in the case of chronic illnesses, digital health tools, media coverage of health issues, and the preferences of new generations of users and healthcare professionals alike. These developments are reflected in the growing heterogeneity of self-employed, salaried or mixed modes of practice, profound changes in the conditions of access to care provided by general practitioners, variable working hours for the latter, new activities, etc... While the French healthcare system still recognizes primary care as the mode of entry and management of the majority of health problems, and the GP as the central, if not exclusive, player in this primary care (as evidenced by the choice of a general practitioner by over 90% of those with social insurance), most of the scientific work carried out in recent years - and, in particular the work carried out by DREES on the panel of practices and conditions of general practice, provide a fragmented vision of the acts and functions of general practice, often limited to private practice and the question of the supply of care. Against this backdrop, it would be useful to carry out a detailed, systematic survey of GPs' activities both inside and outside care, according to their characteristics, conditions, status and areas of practice. Main objective: Describe the activity, visible or unknown, of general practitioners in charge of primary care, according to their practice setting, professional environment, support and partnerships. |

Intervention | Each of these investigator physicians will need to complete an electronic questionnaire on one of the randomly selected activity days, based on the calendars provided. This complex questionnaire will be available on a dedicated app, accessible via the web, allowing completion on a smartphone, tablet, or computer. The questionnaire will aim to identify all professional tasks performed during that day and the time spent on these tasks, including: - **Professional activities by type**: in-person consultations, teleconsultations, home visits, technical procedures, with details on the main reason for the act and its role in the patient's care pathway. - **Written or telephone professional communications**: contact person, purpose, direction of the exchange, medium used. - **Training or medical information search tasks**. - **Administrative or accounting tasks**. Physicians selected to participate in the survey will be compensated for completing the main questionnaire. Descriptive analyses will detail the results based on the profiles of the chosen general practitioners, considering their personal characteristics, modes of practice (liberal, salaried, or mixed), practice locations (private office, health center, public or private health facility), and distinguishing their integration into the healthcare environment (ESP, CPTS, ESCAP, MMG, EHPAD, SAS, etc.), available support systems (DAC, Asalée, medical assistants, etc.), partnerships (SSIAD, GHT, SAU, etc.), and the resources available in their area of practice. The representativeness of the responding physicians will be validated by cross-referencing with available regional reference data (URPS, Health Insurance, FNCS). |

Primary outcome measure | Measured using medical records throughout the study: 1. Levels of care activity within and outside the scope of the GP scheme (consultations, visits, procedures), scheduled and unscheduled care, and use of digital health tools including teleconsultation/tele-expertise. 2. Time spent on training activities, researching medical or medico-social information, management, etc. 3. The number of exchanges relating to professional coordination (specifying sender, receiver, message, channel) and the time spent on these exchanges |
