InterventionPractices in the control group, in common with intervention group, receive a complete disease register for patients with a diagnosis of chronic heart failure due to left ventricular systolic dysfunction. Control group practices do not receive any further pharmacist led support. Intervention practices receive pharmacist medication review for their patients with heart failure.
