Study objectives1. To determine the regional and national availability of open access echocardiography and other specialist diagnostic services for suspected heart failure presenting to primary care
2. To gain an insight into the thinking, currents practices and decision making of GPs on the diagnosis and management of heart failure in primary care
3. To determine GPs', general physicians', geriatricians' and cardiologists' views on the perceived barriers to the diagnosis and management of suspected heart failure in primary care
4. To develop and evaluate the ideal intervention for diagnosis and management of suspected heart failure in primary care

1. Survey of diagnostic facilities.
2. Qualitative data on current practice, decision making and perceived barriers to diagnosis and management of suspected heart failure
3. An integrated care scheme for patients with suspected heart failure (outcomes to be measured will include diagnostic rates, appropriate treatment, quality of life, quality of symptom control, rates of hospital referral, rates of hospitalisation and economic costs)
4. An 'academic detailing' programme for training GPs to manage heart failure
