Background and study aims
An elder’s visit to the emergency department offers an opportunity for healthcare professionals to detect victims of elder abuse and neglect and subsequently initiate an intervention. An elder who is vulnerable and isolated might only leave the house during this situation. The emergency department is frequently the first encounter with healthcare professionals. Considering this, the healthcare professional is in a remarkable position to recognize and address elder abuse and neglected cases. Unfortunately, the detection of elder abuse and neglect in the emergency department is lower than the prevalence estimates. Given the complexities involved in comprehending and reporting abuse, education and training of healthcare professionals is necessary if abuse is to be identified and reported properly. This study aims to develop and evaluate an online elder abuse and neglect module to improve knowledge, attitude and confidence to intervene among emergency department healthcare providers in two different states in Malaysia.
Who can participate?
Emergency department healthcare professionals working under the Ministry of Health including nurses, emergency physicians, medical officers, house officers, and medical assistants, have working experience of at least six months, aged between 18- and 60-years and have not attended any educational program on elder abuse and neglect in the past 12 months.
What does the study involve?
This study will be conducted in two phases. Phase 1 is the development of an online elder abuse and neglect module. This module will be prepared by integrating information from three key sources including: 1) in-depth literature reviews, 2) interviews with a panel of experts, and 3) adaptation and modification of, an existing module that was designed for primary care doctors in Malaysia. Phase 2 will evaluate the effectiveness of the module on knowledge, attitude and confidence to intervene elder abuse and neglect cases among healthcare professionals in emergency department. In the second phase, two emergency departments (in two tertiary hospitals) will be selected for the evaluation of the module using a quasi-experimental design.
What are the possible benefits and risks of participating?
The benefits of participating include improved knowledge and confidence in identifying, diagnosing and managing elder abuse and neglect by healthcare professionals in the emergency department. Participation carries no potential risks.
Where is the study run from?
The study will run from two emergency departments in two tertiary hospitals in Malaysia.
When is the study starting and how long is it expected to run for?
January 2022 until December 2024

Intervention | Two emergency departments (in two tertiary hospitals) will be selected for the evaluation of the module: 1. Control group: will receive a link to a website that contains general elder care. 2. Primary outcome measure | Knowledge, Attitude and confidence to intervene using Elder Abuse Questionnaire (ELSE-Q), a validated Malay language questionnaire, at baseline, immediately post intervention, at 3-months and 6-months follow-up |
Secondary outcome measures | Number of elder abuse and neglect cases intervened and managed during the 6-months follow-up duration. The respondents will be asked how many elder abuse and neglect cases they have intervened and managed for the past 6 months since they completed the module given. The questions will be asked using the online method via the same module website. |

Key exclusion criteria | 1. Have attended any educational program on elder abuse and neglect in the past 12 months. 2. Attachment healthcare professionals (post basic nurses, healthcare professionals in post graduate training) with a duration of employment/attachment of less than 6 months in the respective hospital at the recruitment time. |
