Health condition(s) or problem(s) studiedType 2 diabetes
InterventionDiabetic clinic staff trained by the researcher will make a single telephone call to the telephone number listed with the patient's name and appointment details. This will be conducted 48 hours (two working days) before the scheduled appointment for all patients allocated to the intervention group. If that person is not available or if there is no answer to the single call or if the number is incorrect, disconnected, or out of service, the researcher will attempt to make the call again to a maximum of three times.

A letter which is individually addressed to the patient but follows a standardised script will also be sent to all patients in the intervention group. This letter will contain a stamped addressed envelope and reply slip, telling them when and where to come, where to park, what to bring, who they will see, and what to expect. The letter will be posted 14 days before their initial appointment and again after another seven days for those who do not respond.

Successful contact will be assumed when the participants either answer the phone or by receipt of the patient's reply slip prior to their appointment date. The outcome of the postal and telephone reminder intervention and the identification of the recipient of the message will be recorded on the schedule.

The control group will receive the appointment card as usual, and no further reminders will be sent.

Key inclusion criteria1. Diagnosed with type 2 diabetes (International Classification of Diseases, 10th edition [ICD-10])
2. Aged over 18 years, either sex
3. Attend the diabetes outpatient clinic at Lerdsin Hospital
Key exclusion criteria1. Not diagnosed with type 2 diabetes (ICD-10)
2. Do not attend for follow up at the diabetes outpatient clinic at Lerdsin Hospital, Thailand during the study period 1st May - 30th November 2008
3. Appointment is scheduled less than one working day before their appointment
